Child Registration Details

We provide vibrant youth classes including Tiny Worshipers Jazz, Junior Flow, and High-Energy Fun Hip Hop. Our studio is a gentle, safe space where children discover God's love through dance, building confidence, friendships, and physical coordination.

Personal Details

To enroll your child, please provide their full name, your contact details, emergency phone numbers and their dance or performance history.

Medical & Accessibility

Your child's wellbeing is vital. Kindly list any allergies, health needs, or learning support requirements to help us ensure a safe studio visit.

Studio Policies

Parents must sign our studio behavioral code, tuition terms, and media release consent to help us build a safe dance family community.

Parent/ Guardian Details

Please provide the following details to register your child for our programs.

I authorise for Beautiful Grace Studios to:

  1. Administer standard first aid and necessary parent provided medicationn.

  2. Call an ambulance or transport my child to a hospital in a medical emergency.

  3. Secure urgent treatment if Parents/ Guardians cannot be reached (Parents/ Guardians will accept responsibility for medical/ambulance costs).

Do you give permission for Beautiful Grace Studios to take photos or videos of your child/ren during the holiday program for use on our social media?
